ABSTRACT

ObjectiveTo explore the pharmacodynamic ingredients of Zhenqi Fuzheng granules (ZFG) for immunomodulatory through spectrum-effect relationship analysis, which provides experimental basis for improving the quality standard of ZFG. MethodEighteen batches of ZFG from six manufacturers were collected for analysis. The fingerprints were established by high performance liquid chromatography (HPLC). Acetonitrile (A)-0.1% formic acid aqueous solution (B) were adopted as the mobile phase with gradient elution (0-15 min, 5%A; 15-23 min, 5%-8%A; 23-30 min, 8%-11%A; 30-45 min, 11%-18%A; 45-60 min, 18%-21%A; 60-67 min, 21%-23%A; 67-90 min, 23%-37%A), the detection wavelength was 220 nm. Chemometric analysis such as similarity analysis and hierarchical cluster analysis (HCA) were subsequently used to analyze the similarities and chemical differences among these samples. A cyclophosphamide-induced immunodeficiency mouse model was used to evaluate the immune-enhancing effects of the products from different manufacturers. The spectrum-effect relationship between HPLC fingerprints and the immunomodulatory effects was examined using Spearman bivariate correlation analysis. HPLC coupled with mass spectrometry (HPLC-MSn) was used to identify the spectrum-effect related peaks with electrospray ionization, positive and negative ion modes, and scanning range of m/z 100-1 500. ResultThe HPLC fingerprint of ZFG was established, and twenty peaks with good resolution were selected as common peaks. The results of quality analysis and pharmacodynamic test showed there were significant differences in both ingredients content and immune-enhancing effects of ZFG from different manufacturers. Through spectrum-effect relationship study, twelve peaks were screened as bioactive ingredients peaks. Thereafter, eight peaks among them were subsequently identified by HPLC-MSn. They were salidroside (peak 2), echinacoside (peak 5), calycosin-7-glucoside (peak 6), isomer of specnuezhenide (peak 7), isonuezhenide (peak 9), calycosin (peak 11), nuezhenide G13 or oleonuezhenide (peak 14), and formononetin (peak 18), respectively. ConclusionThere are differences in quality and efficacy of ZFG produced by different manufacturers. Through spectrum-effect relationship analysis, the medicinal ingredients of ZFG for immune-enhancing effects are screened, which can provide reference for the improvement of its quality standard.

ABSTRACT

Objective To study the stability of lumbar spine after transforaminal lumbar interbody fusion (TLIF) surgery combined with a novel articular process fixation system (APFS). Methods Based on the validated finite element model of L3-S1 intact segment (Model A), TLIF surgery was simulated to establish bilateral pedicle screw TLIF model (Model B), right unilateral pedicle screw TLIF model (Model C), APFS combined with right pedicle screw fixation TLIF model (Model D). The range of motion (ROM) of the lumbar spine model and stress distributions on pedicle screws, APFS and interbody fusion cages under different working conditions were observed. Results The overall ROMs of Models B, C, and D under different working conditions were comparable, which were all smaller than those of the physiological model. Compared with Models B and C, the maximum compressive stress of the right pedicle screw and the interbody fusion cage in Model D was the smallest or between Models B and C under different working conditions. Model D had the largest peak stress of APFS and right pedicle screw during anterior flexion. Conclusions APFS combined with contralateral pedicle screw fixation can be used as a novel fixation method for TLIF surgery of lumbar spine.

ABSTRACT

Objective We aimed to assess the association between urinary bisphenol A(BPA)concentrations and gestational age in pregnant women. Methods A total of 248 pregnant women were recruited from a maternal and child care hospital in Shanghai. A questionnaire survey was completed to collect socio-demographic information and spot urine samples were collected during pregnancy. Gas chromatography-tandem mass spectrometry(GC-MS/MS)was used to measure BPA concentrations in urine samples. Linear relationship between urinary BPA level and gestational age was assessed by using generalized additive models. Multivariate regression model was used to evaluate associations of prenatal BPA exposure with gestational age. Results BPA was detected in all the urine samples. Median value and geometric mean of urinary BPA levels were 0.85 μg/L and 1.21 μg/L, respectively. Linear relationship between urinary BPA concentration and gestational weeks was confirmed(non-linear P > 0.05). Positive association between urinary BPA level and gestational age was indicated(regression coefficient, β = 0.19;95%CI:0.04-0.35;P = 0.016). However, it was only observed in girls, stratified by sex of newborns(β = 0.18;95%CI:0.03-0.34;P = 0.020). After stratification by trimester, no significant association was found in the second or the third trimesters. Conclusion Pregnant women are extensively exposed to BPA. Urinary BPA exposure during pregnancy may extend gestational age, especially in girls.

ABSTRACT

Objective:To investigate the clinical effect and short-term and long-term effect of total hip replacement (THA) and hemiarthroplasty of hip (HA) in elderly patients with femoral neck fracture (FNF).Methods:59 elderly patients with FNF in South District of Guang′anmen Hospital, Chinese Academy of Traditional Chinese Medicine from March 2017 to January 2019 were retrospectively analyzed. The observation group ( n=29) was treated with THA; The control group ( n=30) was treated with HA. The operation related indexes, short-term and long-term clinical effect (average follow-up of 24.1 months), hip function improvement, pain and complications were compared between the two groups. Short term and long-term clinical effects: the short-term and long-term clinical effects of the two groups were observed by the Harris Hip Score (HHS) of the first month, the third month, the sixth month, the ninth month and the first year after operation. Results:Both THA and HA were effective on elderly FNF, but the effective rate of THA (96.6%) was significantly higher than that of HA (90.0%), with statistically significant difference ( P<0.05); in terms of operation related indicators, compared with the control group, the observation group had longer operation time [ (104.46±3.24)min vs (84.34±3.64)min], more amount of bleeding [(296.64±15.16)ml vs (281.44±12.16)ml], lower postoperative drainage volume [(101.24±4.15)ml vs (74.56±3.24)ml], while the functional recovery of the observation group was better than the control group by HHS ( P<0.05); the postoperative complications and HHS score of observation group were significantly lower than the control group ( P<0.05). Conclusions:THA has the advantages of low complication, fast functional recovery and better effect. The clinical decision should be based on the patient′s physical condition. THA should be used for elderly patients with good basic condition, good surgical tolerance and more daily activities. HA should be used for elderly patients with poor basic condition, low surgical tolerance and less daily activities.

ABSTRACT

Objective:To apply Botulinum toxin type A (BTX-A) injection for cricopharyngeal achalasia dysphagia guided by ultrasound and electrical stimulation. Methods:A case with cricopharyngeal achalasia dysphagia was reviewed. Results:He ate food almost in normal way six days after injection, and cricopharyngeus was open seven days after injection. Conclusion:BTX-A injection under ultrasound- and electrical stimulation-guide is effective on cricopharyngeal achalasia dysphagia.

OBJECTIVE@#To explore the effect of glucose pretreatment in the rapid rehabilitation surgery of hip replacement patients, and to provide reference for the future clinical treatment.@*METHODS@#From June 2016 to June 2018, 168 patients (100 males, 68 females) were treated with hip replacement. The patients were divided into control group and observation group, 84 cases in each group, aged 25 to 90 (52.05±5.73) years old. Both groups were treated with the concept of rapid rehabilitation surgery, the control group was given traditional fasting water deprivation before operation, and the observation group was given glucose pretreatment before operation. The levels of fasting blood glucose(FBG), C-peptide, fasting insulin(FINS), IgG, IgM, IgA and total lymphocyte count (TLC) were compared before and after operation, and the sensitivity of thirst, nausea, fatigue, sweating, stomach discomfort, anxiety, hunger and dizziness were compared.@*RESULTS@#All the 168 patients were followed up. There was no significant difference in FBG, C-peptide and fins levels between the observation group and the control group (>0.05). The levels of FBG, C-peptide and fins in the observation group were higher than those in the control group (0.05);the serum IgG, IgM, IgA and TLC levels after operation were higher than those before operation, and the observation group was higher than the control group(0.05);the degree of anxiety, hunger and dizziness in the observation group was less than that in the control group (<0.05).@*CONCLUSION@#Glucose pretreatment before hip replacement can relieve insulin resistance, improve immunity and change energy storage under fasting, which can be widely used in clinic.

ABSTRACT

Objective:To explore the effect of CT-guided Botulinum toxin injection into cricopharynx muscle on dysphagia caused by true bulbar palsy. Methods:A case of dysphagia caused by true bulbar palsy was treated with CT-guided Botulinum toxin injection and its therapeutic effect was reported. Results:The patient had dysphagia after brainstem infarction and was diagnosed as true bulbar palsy. After routine rehabilitation of dysphagia and balloon dilatation, her dysphagia relieved, but reappeared three times. Botulinum toxin was injected into the loop pharynx muscle under the guidance of CT, the clinical effect was remarkable, and no recurrence of the disease appeared in the follow-up. Conclusion:CT-guided Botulinum toxin injection into cricopharyngeus is effective on dysphagia caused by true medullary palsy, and the probability of recurrence is small.

ABSTRACT

Objective To investigate the risk factors for cerebral microbleeds (CMBs) in patients with non-disabling ischemic cerebrovascular events (NICE).Methods From January 2017 to September 2018,patients with NICE admitted to the Department of Neurology,the Second Affiliated Hospital of Kunming Medical University were enrolled.The relevant clinical data were collected and the cranial MRI examinations were completed.CMBs were detected by susceptibility-weighted imaging.The demographic and clinical data of the CMBs group and non-CMBs group were compared.Multivariate logistic regression analysis was used to determine the independent risk factors for CMBs.Multivariate linear regression analysis was used to determine the independent influencing factors of the number of CMBs.Results A total of 159 patients were enrolled,including 73 (45.9%) in the CMBs group and 86 (54.1%) in the non-CMBs group.There were significant differences in hypertension,diabetes mellitus,past stroke or transient ischemic attack (TIA) history,carotid atherosclerosis,NICE classification (TIA,mild stroke) and the proportion of patients taking drugs before onset,as well as diastolic blood pressure and white matter Fazekas score between the CMBs group and the non-CMBs group (all P< 0.05).Multivariate logistic regression analysis showed higher diastolic blood pressure (odds ratio 1.047,95% confidence interval 1.016-1.079;P =0.002) and higher Fazekas score (odds ratio 1.825,95 % confidence interval 1.465-2.273;P < 0.001) were the independent risk factors for CMBs.Multiple linear regression analysis showed that there was an independent positive correlation between the white matter Fazekas score and the number of CMBs (r =0.273,P < 0.001).Conclusion In patients with NICE,CMBs were associated with higher diastolic blood pressure level and higher white matter Fazekas score,and the white matter Fazekas score was positively correlated with the severity of CMBs.

ABSTRACT

Panax japonicus is a traditional Chinese medicine,and its principle components have shown certain pharmacological activities for cell damage,aging and cell apoptosis. In order to clarify the pharmacological mechanism and involved metabolic pathways of P. japonicas,the gene expression of Tetrahymena thermophila under P. japonicus treatment was analyzed through high-throughput transcriptome sequencing in this study. Based on the transcriptome analysis,3 544 differentially expressed genes were identified in control group,of which 1 945 genes showed up-regulated expression and 1 599 genes showed down-regulated expression. Under P. japonicas treatment in the experiment group,3 312 differentially expressed genes were screened,of which 1 `493 genes showed up-regulated expression and 1 819 genes showed down-regulated expression. GO enrichment analysis indicated that in control group,the genes in the cells in a series of fundamental biological process were down-regulated,such as DNA replication and protein synthesis; while the signal transduction process and fatty acids oxidizing process were enriched. Whereas in the experiment group,down-regulated genes were mainly enriched in oxidation-reduction,cofactor metabolic process and vitamin metabolic process; up-regulated genes were enriched in signal transduction process and protein modification process. In the analysis using KEGG database,cell cycle pathway was enhanced and autophagy pathway was inhibited under the condition of P. japonicas treatment. Real-time quantitative polymerase chain reaction( RT-qPCR) was used to detect the expression differences between 6 up-regulated and 4 down-regulated genes in related metabolic pathways. The RT-q PCR results and RNA-Seq data were highly correlated and consistent with each other. This study could provide important direction and basis for further study on the mechanism of cell growth regulation with the treatment of P. japonica.

ABSTRACT

BACKGROUND: Morinda officinalis oligosaccharide is the main active ingredient of morinda officinalis extract,which can promote the angiogenesis of ischemic tissue, but the mechanism is unknown. At present, there are two ways for endothelial repair:vascular endothelial cell division or differentiation from vascular endothelial progenitor cells in the peripheral blood. Here, we attempted to explain the pro-angiogenesis mechanism of morinda officinalis oligosaccharide by exploring whether there is a correlation between morinda officinalis oligosaccharide and the biological function of vascular endothelial progenitor cells, thereby providing experimental reference for new drug development. OBJECTIVE: To observe the effect of morindae officinalis oligosaccharide on the proliferation, differentiation and paracrine of vascular endothelial progenitor cells. METHODS: Vascular endothelial progenitor cells were isolated from healthy human peripheral blood, and divided into two groups: control group (without morindae officinalis oligosaccharide) and experimental group (with 0.15 g/L morindae officinalis oligosaccharide), followed by 48 hours of in vitro culture.The proliferation of vascular endothelial progenitor cells was tested by fluorescent staining;the ratio of vascular endothelial progenitor cells expressing CD31 was detected by flow cytometry; and the levels of vascular endothelial growth factor, stromal cell-derived factor 1 and interleukin 8 were analyzed by enzyme-linked immunosorbent method. RESULTS AND CONCLUSION: The percentage of vascular endothelial cells expressing CD34, CD133 or VEGFR- 2 was (84.72±4.34)%. After 48 hours of culture by 0.15 g/L morindae officinalis oligosaccharide, the proliferation rate and the positive expression of CD31 in the experimental group were significantly higher than those in the control group (P < 0.05). The levels of vascular endothelial growth factor, stromal cell-derived factor 1 and interleukin 8 in the experimental group were also higher than those in the control group (P < 0.05). To conclude, morindae officinalis oligosaccharide can promote the proliferation and differentiation of vascular endothelial progenitor cells, and meanwhile, it can stimulate the release of vascular endothelial growth factor, stromal cell-derived factor 1 and interleukin 8 from vascular endothelial progenitor cells through the paracrine pathway. Consequently, it is a potential drug for myocardial ischemic diseases.

ABSTRACT

Objective To investigate the feasibility of 3-D printing technology in the application of proxi-mal tibial epiphyseal injury fracture surgery for teenagers. Methods Clinical materials of the 8 patients with proxi-mal tibial epiphyseal injury fracture who were admitted to our hospital from January 2014 to March 2016 were retro-spectively analyzed. CT data of all patients was collected before the surgery and was used for fracture 3-D recon-struction with Minics software to print out the 1:1 physical model. Meanwhile,surgical simulation was carried out on the 3-D model to design the internal fixation scheme,and all intraoperative operations were implemented in line with the preoperative planning. Postoperative brace immobilization and the early-stage function exercise were taken and growth situation of the affected limbs and fracture healing were observed and recorded. In line with the modified Hohl and Luck knee score standards,knee functional score was taken 12 months after the surgery. Results All pa-tients received postoperative follow-up service for 12 to 18 months,with the average of 15.3 months. All patients were bony union,with the excellent knee score rate of 100%. The full-length axis of two limbs was compared be-tween the postoperative reexamination and no premature skeleton closure. Pronation and supination of the knee,un-stable joints and other complications were investigated. No obvious differences were found among the activity of the affected limbs and 12-month growth length after the surgery and that of the uninjured side. Conclusions 3-D printing technology is a strong clinical guidance to the treatment of proximal tibial epiphyseal injury fracture surgery for teenagers.

ABSTRACT

Objective To study the therapeutic effect of total knee arthroplasty(TKA)in treat-ment of severe osteoarthritis with patella alta.Methods TKA was performed on 13 patients of severe os-teoarthritis with patella alta,to adjust the relative position of the patella by changing the height of the plat-form line in the operation,and measure the length of the perpendicular line from the lowest point of the pa-tella joint surface to the tibial platform line before and after knee replacement,calculate and compare the ratio of the length to the joint surface of the patella(BP index).In addition,the score of the knee joint was evaluated by American special surgery hospital knee joint scoring system(HSS)at preoperative and post-operative three months.Results All the patients'incision got primary healing who were followed up with-out other complications.The BP index and the HSS score was statistically significant before and after sur-gery(P<0.05).Conclusion Total knee arthroplasty can be used to treat severe osteoarthritis with high patella by adjusting the relative height of the patella and correcting the limb alignment,to improve the function of knee joint and the quality of life.

Objective:To examine the effects of chronic intermittent hypoxia(CIH)on the NF-κB,IL-6 and PGE2 level in rats with periodontitis.Methods:32 male SD rats(6 weeks old)were randomly divided into 4 groups(n =8),group A(normoxic control),B (normoxic periodontitis),C(CIH)and D(periodontitis +CIH).Periodontitis model was established in the upper second molars by liga-tion technique and high-glucose diet in the rats of group B and D.The rats in the group C and D were subjected to CIH in a cycle of al-ternative nitrogen and oxygen in a closed chamber.The chamber was filled with nadir and zenith ambient oxygen every 1 20 seconds per cycle for 8 hours per day.The rats were sacrificed and the gingival tissues were examined for the detection of IL-6 and PGE2 expression by ELISA,and NF-κB expression by immunohistochemistry.Results:Histology revealed apical migration of junctional epithetlium and crestal alveolar bone resorption in group B and D,and in the above phenomena of group D was the severest.The content of NF-κB,IL-6 and PGE2 in group B,C,D was higher than that in group A(P <0.05),and in group D was the highest(P <0.05).Conclusion:Chro-nic intermittent hypoxia can aggravate the inflammation of periodontitis.

ABSTRACT

Meiotic recombination is carried out through a specialized pathway for the formation and repair of DNA double-strand breaks (DSBs) made by the Spo11 protein. The present study shed light on the functional role of cyclin, CYC2, in Tetrahymena thermophila which has transcriptionally high expression level during meiosis process. Knocking out the CYC2 gene results in arrest of meiotic conjugation process at 2.5-3.5 h after conjugation initiation, before the meiosis division starts, and in company with the absence of DSBs. To investigate the underlying mechanism of this phenomenon, a complete transcriptome profile was performed between wild-type strain and CYC2 knock-out strain. Functional analysis of RNA-Seq results identifies related differentially expressed genes (DEGs) including SPO11 and these DEGs are enriched in DNA repair/mismatch repair (MMR) terms in homologous recombination (HR), which indicates that CYC2 could play a crucial role in meiosis by regulating SPO11 and participating in HR.

ABSTRACT

Objective:To evaluate the efficacy of the resection of nasal inverted papilloma (NIP) under endoscope.Methods: A total of 100 patients with NIP from April 2008 to December 2013 in our hospital were randomly divided into endoscopic surgery group (endoscopic group) and conventional surgery group (conventional group), each with 50 cases. All patients were followed up for 2 years, whose recurrence, complications and satisfaction were observed. Results: The patients were followed up for 2 years, and the recurrence rate between the two groups was not statistically significant (x2=0.96,P>0.05). The traditional group had a higher incidence of complications than the endoscopic group, and the difference was statistically significant (x2=4.16,P<0.05). Endoscopic patients’ satisfaction was higher, and the difference was statistically significant (x2=5.02,P<0.05).Conclusion: The resection of (NIP) under endoscope can reduce the incidence of complication, improve patient satisfaction, and will not increase the recurrence rate, which is an effective means of treatment to the NIP.

ABSTRACT

<p><b>BACKGROUND</b>Intravascular ultrasound (IVUS) examination can provide useful information during endovascular stent graft repair. However, its actual clinical utility in thoracic endovascular aortic repair (TEVAR) for type B aortic dissection (type B-AD) remains unclear, especially in complicated aortic dissection. We evaluated the effect of IVUS as a complementary tool during TEVAR.</p><p><b>METHODS</b>From September 2011 to April 2012, we conducted a prospective cohort study of 47 consecutive patients with "complicated" type B-AD diagnosed. We divided the patients into two groups: IVUS-assisted TEVAR group and TEVAR using angiography alone group. The general procedure of TEVAR was performed. We evaluated the perioperative and follow-up events. Patient demographics, comorbidities, preoperative images, dissection morphology, details of operative strategy, intraoperative events, and postoperative course were recorded.</p><p><b>RESULTS</b>A total of 47 patients receiving TEVAR were enrolled. Among them (females, 8.51%; mean age, 57.38 ± 13.02 years), 13 cases (27.66%) were selected in the IVUS-assisted TEVAR group, and 34 were selected in the TEVAR group. All patients were symptomatic. The average diameter values of IVUS measurements in the landing zone were greater than those estimated by computed tomography angiography (31.82 ± 4.21 mm vs. 30.64 ± 4.13 mm, P < 0.001). The technique success rate was 100%. Among the postoperative outcomes, statistical differences were only observed between the IVUS-assisted TEVAR group and TEVAR group for total operative time and the amount of contrast used (P = 0.013 and P < 0.001, respectively). The follow-up ranged from 15 to 36 months for the IVUS-assisted TEVAR group and from 10 to 35 months for the TEVAR group (P = 0.646). The primary endpoints were no statistical difference in the two groups.</p><p><b>CONCLUSIONS</b>Intraoperative IVUS-assisted TEVAR is clinically feasible and safe. For the endovascular repair of "complicated" type B-AD, IVUS may be helpful for understanding dissection morphology and decrease the operative time and the amount of contrast used.</p>

ABSTRACT

Objective:To investigate the expression of tumor necrosis factor-α(TNF-α)and soluble intercellular adhesion molecule -1(sICAM-1)in blood serum and gingival tissue in rat periodontitis model in normoxia and chronic intermittent hypoxia environment. Methods:32 male SD rats were randomly divided into 4 groups(n =8):normoxia control group(group A),normoxia periodontitis group(group B),hypoxic control group(group C)and hypoxia periodontitis group(group D).The periodontitis models were estab-lished by ligating the bilateral maxillary second molar and raised by periodontitis diet.The rats in hypoxia groups were raised under chronic intermittent hypoxia environment,while those in normoxia groups were raised under normoxia environment.After 8 weeks, plaque index(PLI),bleeding index(BI)and attachment lost(AL)were measured,TNF-αand sICAM-1 concentrations in blood serum and gingival tissue were measured by ELISA.Results:TNF-αand sICAM-1 concentrations in blood serum and gingival tissue in group A were higher than those in group B,C(P <0.05);in group D were lower than in group B and C(P <0.05).TNF-αand sICAM-1 levels in blood serum and gingival tissue were positively correlated with AL(P <0.05).Conclusion:In chronic intermittent hypoxic environment TNF-αand sICAM-1 may aggravate periodontitis,and promote the inflammatory response to peripheral vascular system.

ABSTRACT

Realgar-containing Niuhuang Jiedu tablet (NHJD) has been applied in clinic for more than 800 years. However, because realgar contains arsenic (As), it has aroused wide concerns and controversies both at home and abroad. Currently, there are two misunderstandings about realgar-containing Chinese patent medicines. First, some people exaggerated realgar's toxicity as that of arsenic. Second, they recommended to remove realgar from traditional Chinese medicine compounds. In this paper, the authors summarized the advance in studies on NHJD, and proposed different opinions: (1) It is inappropriate to take total As as the index in safety evaluation of NHJD. (2) The toxicity of NHJD is dependent on the dose and duration of administration. (3) Realgar is an active ingredient of NHJD, and shall be deeply studied. Classic realgar-containing traditional Chinese medicine prescriptions, such as Niuhuang Jiedu tablet, shall be evaluated with rigorous modern scientific basis, with the aim to guide rational and safe application.

ABSTRACT

ObjectiveTo compare the safety and efficacy of ligature between traditional ligation way in gastrectomy.Methods60 patients with gastrointestinal cancioma were selected and randomly divided into two groups,30cases of ligature(treatment group),30 cases of ligation (traditional group).ResultsThe operative time,blood loss,postoperative hospital stay,complications and treatment costs was compared between the two groups.Compared with the traditional group,there were shorter treatment time,reduced bleeding,shorter hospital stay,fewer complications,reduced treatment costs in treatment group.ConclusionLigature gastrectomy is safe and effective.
